Our Well Being Co-ordinator and team of Assistant’s are here to assist and enable residents to have a fulfilling and meaningful experience whilst living at The Chace. Our aim is to encourage and continue previously enjoyed favourite activities and interests, whether that is visiting the local pub each week, playing the piano or continuing a passion for gardening. We also incorporate ordinary life activities to maintain independence and structure to the lives of the residents. Special events and celebrations are held regularly and families and friends are always welcome.
When you first arrive, the Wellbeing team will talk to you about what you like to do. They will ask you to share those life experiences that are important to you and you wish to remember. This enables us to build up a picture of you as a person and help us to value those experiences, recording them in your own Life Memory Book which will ensure we always have an understanding of your needs, wishes and aspirations.
Our Wellbeing team and Carers continuously update our bespoke Care Mapping programme, which focusses on the experience of each resident, responding quickly to any changing needs and provides truly person centred care for every resident. It also enables us to give feedback to our families on a monthly basis.
